UPDATE: Possibility for Children Under 16 to Participate in the Walmart COVID-19 Vaccination Clinic

Good afternoon Cedar Cliff and Red Land High School families,
 
As you may be aware, the U.S. Food and Drug Administration may authorize the Pfizer COVID-19 vaccine for children 12 to 15 years old, a decision that could come early next week prior to our vaccination clinics. The vaccine is currently authorized only for people age 16 and older.
 
If your child is not yet 16 and you would like them to receive the vaccine should the FDA approve Walmart Pharmacy to administer it, please complete the pre-registration form by Friday, May 7, at 1 p.m.  
 
It is important to note that Walmart Pharmacy will determine if children under 16 may participate in their clinic. Completion of the form does not guarantee your child will receive the vaccine.
 
 
The above form may also be used to pre-register your child if they are 16 and over and you are interested in them receiving the vaccine during the Walmart Pharmacy COVID-19 Vaccination Clinic at Cedar Cliff and Red Land High Schools. Participation in this clinic is voluntary.

Good evening Cedar Cliff and Red Land High School families,
 
The West Shore School District will host a Walmart Pharmacy COVID-19 Vaccination Clinic for any interested students age 16 and over at Cedar Cliff and Red Land High Schools. Participation in this clinic is voluntary.
 
The first dose of the Pfizer vaccine will be administered by Walmart Pharmacy staff on Thursday, May 13, and the second dose will be provided on Thursday, June 3, to complete the vaccine series. Doses will be administered at school during the school day. More details on time and specific location will be shared once we know how many students will be participating.
 
Parents/guardians interested in pre-registering their West Shore student to receive the vaccine must pre-register by Friday, May 7, at 1 p.m. Current students only. 
 

There is no out of pocket cost to you or your student to participate in the clinic; however, for your student to be vaccinated, parents/guardians will need to complete the attached Patient Forms and have their child bring them to the clinic. Parents/Guardians must sign Section C of the Vaccine Administration Record and the Minor Consent Form for any students under 18 years-old. Walmart Pharmacy cannot administer the vaccine to minors during the clinic unless the Minor Consent Form is completed. Additionally, the Insurance Attestation form should be filled out with current insurance information.
 
Please Note:
  • This clinic will be facilitated by Walmart Pharmacy staff. Participation is voluntary.
  • Those scheduled for an appointment should NOT report to school or for the vaccine if they or someone in their household is exhibiting any sign or symptom of COVID-19, or if they were identified as a close contact of someone diagnosed with the virus and currently in quarantine.
  • The West Shore School District is not liable for any injury, loss or damage that may occur following vaccination or as a result of participation in the Walmart Pharmacy COVID-19 Vaccination Clinic.
